What Is No-Exam Life Insurance?
No-exam life insurance is exactly what it sounds like: a life insurance policy you can get without undergoing a traditional medical exam (which typically involves a blood draw, urine sample, blood pressure reading, and height/weight measurement).
Instead of lab work, insurers use health questionnaires, prescription drug history, motor vehicle records, and database checks to assess your risk profile. This speeds up the approval process dramatically — from weeks to minutes in some cases.
There are three main types:
Simplified Issue Life Insurance
Simplified issue policies require you to answer a series of health questions, but no physical exam. Questions typically cover:
- Have you been diagnosed with cancer, heart disease, or HIV in the past 5–10 years?
- Have you been hospitalized in the past 12 months?
- Do you use tobacco?
- Have you been denied life insurance in the past?
Based on your answers and database checks (MIB, pharmacy records, MVR), the insurer makes an underwriting decision — often within 24 hours.
Coverage available: $25,000 to $1,000,000 (some carriers go up to $2 million)
Who qualifies: Relatively healthy applicants who can answer the health questions favorably
Premium vs. standard: Typically 10–20% higher than a fully underwritten policy
Guaranteed Issue Life Insurance
Guaranteed issue (GI) policies accept anyone within the eligible age range — no health questions, no medical exam, no possibility of rejection. If you're within the age bracket (usually 45–85), you're approved.
The trade-off is significant:
- Lower coverage limits: Most GI policies cap at $25,000–$30,000
- Higher premiums: You're pooled with the sickest applicants, so everyone pays more
- Graded death benefit: Most GI policies include a 2-year waiting period. If you die of natural causes within the first 2 years, beneficiaries receive only a return of premiums plus interest — not the full death benefit. (Accidental death is typically covered from day one.)
Who should consider it: People with serious health conditions who cannot qualify for any other coverage, or older adults seeking final expense coverage only.
Accelerated Underwriting
This is the fastest-growing category and represents the future of life insurance. Carriers use algorithmic underwriting — analyzing thousands of data points including pharmacy records, credit data, motor vehicle records, and public records — to approve applicants without an exam.
Unlike simplified issue, accelerated underwriting policies are fully underwritten at standard (or even preferred) rates if the data looks clean. The difference: you get approved in minutes instead of waiting 4–6 weeks for a traditional exam and review.
Many major carriers now offer accelerated underwriting up to $3,000,000 for applicants under 60.
Coverage available: Up to $2,000,000–$3,000,000
Approval speed: Same day to 48 hours
Rates vs. traditional: Often identical to fully underwritten rates
Is No-Exam Life Insurance Worth It?
The answer depends on your situation:
Yes, if:
- You're healthy and want fast coverage without scheduling a medical exam
- You're a procrastinator — removing friction increases the chance you actually get covered
- You need coverage quickly (new baby, new mortgage, divorce settlement requiring coverage)
- You have mild health issues that might complicate traditional underwriting
No, if:
- You have serious health conditions — traditional underwriting through specialty carriers may actually offer better terms
- Maximum coverage at minimum cost is your priority — traditional underwriting at preferred rates will usually be cheaper
- You're over 50 and want $1M+ in coverage — traditional underwriting may open more options
Sample No-Exam vs. Traditional Rate Comparison
For a healthy 40-year-old female, $500,000, 20-year term:
- Traditional (fully underwritten): ~$33/month
- Accelerated underwriting (no exam): ~$33–$36/month
- Simplified issue: ~$38–$45/month
- Guaranteed issue: Not available at this coverage level
For most healthy applicants, accelerated underwriting delivers near-identical rates with dramatically faster approval.
The Bottom Line
If you're under 60, reasonably healthy, and want coverage quickly, accelerated underwriting no-exam policies offer an excellent combination of speed and value. You'll likely get approved at competitive rates without ever scheduling a paramedical exam.
